Applies to licensed nursing homes only (He-P 803). NH's separate assisted-living-residence/residential-care rule (He-P 804) is genuinely ambiguous: its fine schedule references a possible 'change in... medical director,' but the operative notice-of-change provision it cites (He-P 804.08(i)) only actually addresses the administrator — suggesting the role may be optional there rather than a codified requirement.
