Notable absence, not an oversight: Maryland's nursing-home regulation requires resident-care policies to be developed 'with the advice of the principal physician or medical staff or medical director, IF APPLICABLE' (COMAR 10.07.02.13) โ€” a medical director is one option among several, not mandatory โ€” and otherwise imposes only per-resident attending-physician duties (visits, orders; COMAR 10.07.02.14), not a facility-level director designation. A federal medical-director requirement (42 CFR ยง 483.75(i)) still applies separately to any Maryland nursing home that is Medicare/Medicaid certified.
