Arizona: medical director for a long-term care / assisted living / group home

A physician responsible for the coordination of medical services provided to residents.

• Ensure each resident has an attending physician
• Ensure an attending physician (or a designated alternate) is available 24 hours a day
• Ensure a resident receives a physical examination at least once every 12 months after admission
• Ensure influenza and pneumonia vaccination is offered to each resident at least once every 12 months, absent an applicable exemption

Confirmed for licensed nursing care institutions specifically (Ariz. Admin. Code R9-10-413, Article 4). Arizona's separate assisted living facility rules (Article 8) were only confirmed to require medical/nursing/behavioral-health services be provided 'under the direction of' the appropriate licensed professional — no distinct 'Medical Director' title was confirmed there, so don't assume this extends to a standalone assisted living facility or group home.
